• Lost passport/damaged etc. - FIR i.e. Police Station Complaint, affidavit, application stating the facts place/date of birth (birth certificate), stay proof fees of Rs.2500 (all documents in 5 sets).
• Change from maiden name to married name - valid maiden name passport, marriage certificate, stay proof, 7 photographs, affidavit fees of Rs.300/- (all documents in 2 sets).
• Valid old passport within 6 months of final expiry or within 1 year after final expiry valid passport, stay proof, 7 photographs. (For renewal)
• Signature of applicant is required in ballpoint pen.

Fee is to be paid along with the application form either by bank draft drawn on State Bank of India, Market Yard Branch, Pune (code No. 6117) in favour of the Passport Office or in cash. In case of Bank Draft full name of the applicant should be written on the reverse of bank draft.

Passport shall be delivered only to the applicant or dispatched by registered post acknowledgement due to the address given in the application form.

List of Applicants Entitled to Emigration Check Not Required Stamp (ECNR). This list is prepared by the Ministry of Labour.

• Persons going abroad in managerial capacity in Hotels/Restaurants. Tea-House or other places of public resort etc. possessing specialized Degrees in these fields.
• All Gazetted Government servants.
• All Income-Tax payers (including Agricultural Income-Tax Payees) in their individual capacity, Proof of assessment to Income-Tax and actual payment of income tax for last three years to be insisted upon, and not merely payment of advance tax.
• All professional Degree holders, such as Doctors holding M.B.B.S. Degrees in Ayurved or Homoeopathy, accredited journalists, Engineers, Chartered Accountants, Cost Accountants, Lecturers, Teachers, Scientists, Advocates, etc.
• Spouses and dependent children up to the age of 24 years of category of persons list from (b) to (d).
• All persons who have been staying abroad for more than three years, (the period of the three years could be either at a stretch or broken) and spouses and children up to the age of 24 years of such persons.
• Seamen who are in possession of CDC and Sea Cadets.
• All holders of Diplomatic/Official Passports.
• Dependent children of Parents whose passport is classified as E.C.N.R. in the case of such children E.C.N.R classification to be restricted until they attain 24 years of age.
• Persons holding permanent immigration Visas, such as the visas of UK, USA and Australia.
• Persons holding Graduate or higher Degrees.
• Persons holding 3 years Diploma equivalent to degree from recognized Institution like polytechnics.
• Nurses possessing qualifications recognized under the Indian Nursing Council Act, 1947.
• All persons above the age of 60 years.
• No emigration clearance is required for visiting Bangladesh, Pakistan and all countries in Europe, (excluding CIS), North America, New Zealand, Australia and Japan.
• POEs will also grant emigration clearance to certain categories of skilled/semi-skilled workers for a period of six months from 4-10-91 when they apply through authorized recruiting agents with passport containing valid employment visa.

• Birth certificate issued by a Municipal Authority or district office of the Registrar of Births & Deaths.
• Date of birth certificate or nationality certificate from the school last attended by the applicant or any other recognized educational institution
• Old passport
• Affidavit sworn before a Magistrate stating nationality and date/place of birth, as per specimen in Annexure ‘F’.
