Passport Application
Q. How do you apply for a passport?
A.

An application for a passport may be submitted personally OR through a representative OR by registered post.

Fresh Passport
Q. What documents do you have to attach while applying for a fresh passport?
A. (All original documents should be produced for verification and same will be returned after verification.) Enclose 2 sets containing copy of proof of:
  • Two photocopies of applicant’s ration card OR any one of the following documents, appointment letter of reputed companies on letterhead/water tax/telephone bill/electricity bill/running bank account etc./Income Tax assessment Order of last three years/Election Commission I.D. card.
  • Attested copy of citizenship document if applicant is a citizen of India by Registration or Naturalisation.
  • If the applicant has not continuously stayed at the present address for the last two years, please photocopy the “Personal Particulars form” and enclose and additional set (in duplicate with photographs) for each additional place of residence.
  • Govt./Public Sector/Statutory body employees should submit “No Objection” Certificate in original (Annexure E).
  • If the applicant is eligible for “ECNR” attach attested copy of supporting document (see para 4).
  • If the applicant was repatriated at Govt. cost enclose documents to show that expenditure incurred by the Govt. on his/her repatriation has been fully refunded to the Govt. of India, Ministry of External Affairs.
  • If the applicant was deported to India, give details of Emergency Certificate/Passport.
  • Date of birth proof - birth certificate or school leaving certificate.
  • 7 photographs.
  • Deed Poll affidavit/marriage certificate for change of name on passport after marriage.
  • Signature of the applicant is required in ballpoint pen.

(All documents in 2 sets) Original documents may be certified at the counter.

Afrer 10 years
Q. What are the documents that you attach when applying for a passport after 10 years?
A. Old passport and photocopy of first four and last four pages.

Minor's Passport
Q. What are the requirements when applying for a Minor’s passport (below 15 years)?
A.
  • Consent of both the parents. Copy of mother’s and father’s passport, consent letter from father and mother and both parents should sign.
  • In case the parent(s) are outside India the consent can be obtained in the form of affidavit duly attested by the Indian mission abroad.
  • Affidavit by legal guardian (Annexure C)
  • Affidavit by 2 persons who know the legal guardian as well as minor (Annexure D).

Emergency Passport
Q. What is the procedure in the case of an Out of Turn Issue of Passport (emergency passport)?
A.

An applicant desiring to obtain his passport out of turn in view of any emergency should submit necessary documentation substantiating the urgency. In case the applicant wishes to obtain his passport in less than four weeks from the date of submission of the application form a verification certificate as per the specimen at Annexure ‘G’ should also be submitted along with the documentation of urgency. The Passport Officer shall retain the right to verify in writing the authenticity of the Verification Certificate from the official who has issued it. All applicants seeking a passport out of turn are advised to meet the Passport Officer concerned after submitting their applications with the documentation as specified. Police verification (i.e. they will come to your house to verify the given details) shall be done in all cases of issue of passport.

Change of Name
Q. What are the documents required in the case of changing of name?
A.
  1. Following Marriage, Remarriage or Divorce A woman applying for first time for a passport in her married name for change of name/surname in existing passport on account of marriage OR for substitution of father’s name with husband’s name, must furnish an attested copy of marriage certificate issued by Registrar of Marriage OR an affidavit from the husband and wife. Divorcees applying for change of name OR for substitution of husband’s name with father’s name existing passport must furnish: Divorce deed duly authenticated by court OR Deed poll/sworn affidavit (Annexure ‘A’) Remarried ladies applying for change of name/Husband’s name must furnish: Divorce deed/death certificate as the case may be in respect of her first husband, and Documents as at (a) above relating to second marriage.
